Owner SMS consent example

This is a blank, read-only example of the Merchant Eyes Order Alerts authorization form. It does not enroll anyone and is not evidence of completed consent.
The actual form requires the Merchant Eyes owner to sign in. The owner enters their own mobile number, checks the initially unchecked consent box, and selects “Record my SMS consent.” The completed record is kept privately with the disclosure version, authenticated owner identity and server timestamp.
